Shot with a Helios 44-2 58mm f/2 reverse mounted on 40mm of extension tubes (this was basically a test shot for my custom DIY reverse ring, more info on the stetup here: [link]).

Aperture was at f/11, lighting with a flash on the hotshoe with a quick and dirty piece of cardboard attached as a large "bounce card" to get some light in front of the lens. Need to make a proper macro flash light diffuser some day.

Yes you need. Whites are too 'hard'.
